FINANCIAL READINESS FOR LICENSING
Proof of Financial Ability to Operate (PFA) helps AHCA assess whether an applicant has the resources to operate. Often called a financial affidavit, the filing can involve financial schedules, projections, and supporting evidence. SDC helps organize the administrative work around your provider’s requirements.

YOUR QUESTIONS, ANSWERED
Financial affidavit is a common informal label. The applicable AHCA requirement is Proof of Financial Ability to Operate. Start with the current form and checklist for your provider type; the filing may require multiple schedules and supporting records.
Rule 59A-35.062 requires PFA for initial and change-of-ownership applications for specified providers. It is not a routine requirement for every renewal. AHCA may require it when a licensee shows financial instability. Other provider-specific application events can also carry financial requirements.
The rule specifically requires CPA compilation and signatures for Home Health Agencies, Home Medical Equipment Providers, and Health Care Clinics. Do not assume that this requirement is identical for every provider. Review the current provider-specific form with your accountant.
The standard PFA rule calls for two-year projections. A limited change-of-ownership exception may allow omission of the second year. Nurse registries use a separate form, AHCA Form 3110-7004A; follow its current instructions rather than applying one projection period to every filing.
A change of ownership can involve a change in the licensed entity evidenced by a new FEIN or TIN, or a transfer of at least 51% of ownership or controlling interest, subject to statutory exceptions. The CHOW application must reach AHCA at least 60 days before the change. Review the transaction with your advisors early and coordinate the applicable financial filing.
SDC organizes documents, coordinates information with your CPA, and supports administrative follow-up within the agreed scope. Professional accounting work and required CPA compilation or signatures are arranged separately. SDC does not certify financial statements or promise a licensing result.
